Home & Community Care
The Tasmanian Home and Community Care Program is funded by the Tasmanian Government to provide basic community care services and support to people under 65 years or Aboriginal and Torres Strait Islander people aged less that 50 years of age living with a disability and their carer. The program allows people to continue living independently at home if their independence is at risk due to health event, an ongoing health issue or condition, or they are not eligible for NDIS.
Commonwealth
Home Support Program
Commonwealth Home Support Program is funded by the Commonwealth Government. This Program is deigned to help frail Aged over 65 or Aboriginal and Torres Strait Islanders over 50 years who manage well at home independently but require extra support to continue to maintain independent living in their own homes. Services this program offers includes social support, transport, personal care, domestic assistance, home maintenance, respite, and specialised support services.
